Requirements for Entering PreK 1. State issued birth certificate 2. Updated Immunizations:  4 DTP   3 Polio   1 MMR   3 Hepatitis B    2 Hepatitis A      1 Varicella 3. Proof of residence: Parent/Guardian must have a bill or lease showing their name and physical address. 4. Students must be potty trained. Vaccinations:  Pre-K and all new students will need to provide a copy of their shot records. Students entering the 7th grade are required by law to have their TDAP vaccination.  These are available at the local Health Department.
